PRICING 

Consultation: Complimentary 

I am happy to provide a 15 minute phone consultation to help determine if a physical therapy evaluation is appropriate for your child. Please reach out to schedule. 

Initial Evaluation (1 hour): $200 

The first session will consist of a full evaluation to determine a plan of care that is tailored to your child and your family. 

After completing the evaluation, I will educate you on interventions and recommendations to implement at home. We will determine a follow up schedule as needed. 

Follow Up Treatment Sessions (50 minutes): $160 

During each session, I provide individualized therapeutic intervention paired with ongoing, intentional guidance for parents/caregivers to support progress beyond our time together. 

Insurance Information: 

I am in network with Colorado Medicaid. 

I am out of network with all other insurances. I can provide a super bill upon request for you to submit to your insurance company for potential reimbursement. 

All payments are due at the time of service and provision of a superbill does not gauruntee a reimbursement from your insurance company.
